OBJECTIVE
The Transitional Care Center at Cincinnati Children’s faces complex barriers to safely transitioning children on long-term mechanical ventilation home.
As a foundation for continued work, Live Well needed to consolidate what we learned into an accessible resource to equip future teams to understand the topic, challenges, and opportunities.

Caring for a child on long-term mechanical ventilation is very demanding for caregivers. These medically complex kids need 24/7 awake, alert care, which comes with a huge learning curve and a difficult search for home nursing, often resulting in long discharge delays.
To help Cincinnati Children's transition children home safer and more efficiently, our team first met with dozens of health care professionals and families to understand the patient population and the associated pain points.
In-unit observation, 1:1 interviews, and focus group interviews
After our interviews, we chose to create a physical guide to communicate our learnings with teams that would continue this work in the future.
LTMV is complex, emotional, and intimidating without prior knowledge. An easy-to-understand, digestible artifact is key to onboarding future teams and can be adapted for other audiences.
A physical guide also makes these concepts feel more tangible, human, and accessible.
DESIGN SYSTEM
To further humanize the content of the guide, bright colors, playful illustrations, and family-focused word choice were carefully selected.
ITERATING
Through team-wide critique sessions, the information hierarchy was refined for clarity, medical content was reviewed for accuracy, and print specifications were adjusted for the final professional print.
Further refinement focused on finer details, from copy editing and illustration selection to spacing, color values, justification, and hyphenation.
